Summary
This Portuguese randomised trial, led by the University of Aveiro, will bring family members and other informal carers into pulmonary rehabilitation programmes alongside patients with chronic lung diseases. The comparison is against standard rehabilitation without carer involvement. The main outcome is carers' preparedness for caregiving after the programme.
Objectives
Test whether including informal carers in pulmonary rehabilitation improves their preparedness for caregiving.
Compare carer-inclusive pulmonary rehabilitation with standard pulmonary rehabilitation for people with chronic respiratory disease.

The SMaRT study in Ontario, Canada, evaluates a model in which nurses coordinate the care of patients with multiple chronic conditions using home telemonitoring. Patients send home measurements through the Medly platform so the care team can spot problems between visits. The study compares early death and readmission outcomes with usual care.
HEART Camp was a nurse-led feasibility trial at the University of Nebraska, funded by the National Institute of Nursing Research. Participants with heart failure exercised in small groups guided by a nurse and an exercise physiologist, with support tapering off over six months to build self-management. Results were published in nursing and cardiopulmonary rehabilitation journals.
